Fatal fork fall

OSHA is investigating the death of a man who fell from a three sided box on the forks of a fork-truck or telehandler at a site in Carthage, Missouri.

The Occupational Safety and Health Administration (OSHA) is investigating the death of a man who fell from a three sided box resting on the forks of a fork-truck or telehandler at a site in Carthage, Missouri.
According to OSHA, the man, Luis Estrada, 29, was working from the box when it slipped, he fell to the ground from a height of around 3.5 metres and the box landed on top of him.

He was rushed to hospital but declared dead on arrival. He was working for OSHA’s Kansas office has Launched an investigation into Estrada’s employer, Mundo Hernandez Construction and the general contractor on the site - Arco Construction.

The incident occurred on the site of a new residential care centre on Saturday, November 21st.
